Our Simple 3-Step Referral Process

1. Submit Your Referral

Look below to complete our secure, 5-minute intake form. 

2. Getting to Know the Participant

Within 24–48 hours, we contact the participant and/or referrer to better understand:

Goals and aspirations

Current supports

Cultural and community connections

Communication preferences

Support needs and risks

Preferred support worker characteristics

3. Co-Designing Supports

Together we:

Develop a tailored support plan

Match participants with suitable workers

Establish service agreements

Create consistent schedules

Review supports regularly as needs change

This reinforces co-design rather than allocation.
