At Bamford Community Care, we pride ourselves on a responsive, collaborative, and stress-free intake process. Whether you are a Support Coordinator looking for a reliable, long-term provider for a complex participant, a hospital discharge planner requiring urgent clinical nursing care, or a family member seeking support for a loved one, we are here to streamline the transition.

We accept referrals for NDIS participants who are Agency Managed, Plan-Managed or Self-Managed.

Our Simple 3-Step Referral Process

1. Submit Your Referral

Look below to complete our secure, 5-minute intake form. 

2. Intake Review & Consultation (Within 24–48 Hours)

Our Co-Founder and Operations Manager, Krys, will personally review the referral to assess capacity and compatibility. We will contact the referrer and the participant to discuss goals, scheduling, and preferred support worker characteristics.

3. Service Agreement & Commencing Support

Once approved, we collaboratively hand-select the support team with the participant to ensure true choice and control. We establish the Service Agreement, finalise schedules, and match the client with consistent, long-term staff to begin their journey with us.
